1 INTRODUCTION. Thiopurines are immunosuppressive drugs that have an important role in the treatment of inflammatory bowel diseases (IBD), both as monotherapy and as combination treatment with biologics. 1-3 Yet many patients have to discontinue treatment in an early phase due to ineffectiveness or side effects. Leucopenia as a result of thiopurine use is a frequent reason for dose …

Thiopurines are prodrugs ‒ to be of any benefit to the patient, they need to be converted upon absorption. Excessive methylation via thiopurine methyltransferase (TPMT) frequently causes therapeutic failure. Allopurinol reduces excessive 6-methyl-mercaptopurine (6-MMP) while enhancing 6-thioguanine (6-TGN) levels. The aim of this study was to evaluate clinical, metabolic and endoscopic impact of allopurinol in combination with low-dose thiopurine in IBD. Metabolism of thiopurine drugs--azathioprine, 6-mercaptopurine, and 6-thioguanine--has provided a powerful pharmacogenetic model incorporating polymorphism of the enzyme thiopurine methyltransferase (TPMT) and the primary active metabolite, thioguanine nucleotide (TGN).

They work by reducing inflammation in the intestine. For this study, the authors revisited the same dataset, looking at nearly 1,500 IBD cases from 47 countries. TPMT is best known for its role in the metabolism of the thiopurine drugs such as azathioprine, 6-mercaptopurine and 6-thioguanine. TPMT catalyzes the S-methylation of thiopurine drugs. IMURAN (azathioprine), an immunosuppressive antimetabolite, is available in tablet form for oral administration. Each scored tablet contains 50 mg azathioprine and the inactive ingredients lactose, magnesium stearate, potato starch, povidone, and stearic acid. Azathioprine is chemically 6- [ (1-methyl-4-nitro-1H-imidazol-5-yl)thio]-1H-purine.

Recent research has shown that genetic variants in NUDT15 also affect thiopurine toxicity. 1,2 NUDT15 can only be tested through the genotype test. Therefore, both the genotyping assay and the enzyme assay are considered complementary and are recommended.

The inactivation of thiopurine drugs is catalyzed in part by thiopurine methyltrasferase … 1 INTRODUCTION. Thiopurines are immunosuppressive drugs that have an important role in the treatment of inflammatory bowel diseases (IBD), both as monotherapy and as combination treatment with biologics.

Drug Drug Description; Epstein-Barr virus (EBV) seronegativity in patients commencing thiopurines has long been a concern and it was identified in 2002 that treatment in this setting was associated with a small increased risk of EBV-positive lymphomas.27 More recently in the CESAME observational cohort, thiopurine treatment had an HR of 5.28 for developing lymphoproliferative disorders with EBV infection being a further risk factor.28 The risk remains small and as yet, avoidance of these drugs in seronegative 6-Mercaptopurine (6-MP) and its prodrug, azathioprine (AZA), have been used in the management of Crohn’s disease (CD) for more than half a century [ 1.
